AONE Needs Your Exemplars!

AONE Guiding Principles for Future Care Delivery have been developed as a framework for the design of care delivery models. The related Operational Definitions further clarify these Guiding Principles and are offered as a supplemental tool to aspiring model designers.
 
The Patient Care Delivery Task Force invites you to provide real world stories from your current practice settings that bring these principles to life. Your stories will inform our continuing work.
 
In order to guide you in telling your story, please review the principles and select which one(s) best describe, or “fit” your exemplar story. Then, respond to the prompts that follow.
 
All submissions will be reviewed for relevance and length, and may be posted on the AONE web site as part of the Guiding Principles Toolkit and/or used in conjunction with other AONE Future Care Delivery initiatives.  Submitting your contact information is optional, but this information will be accessible only to AONE members.
