C-HAWAII, Communication among caregivers, High risk medications, Acquired infections, Wrong site/ patient/procedure/surgery, Accurate medications, Identification of patients, and Infusion pumps. If patient safety goals were reached with 100% compliace one employee would receive an all-expense paid trip for two to Hawaii. Every 3 weeks the campaign highlighted a different safety goal with a fun giveaway. Hospital newsletters had updates and reminers about patient safety issues. |

Commitment to a clinical practice environment which engenders patient safety. Ingrain patient safety measures into the culture on a daily basis. Compliance with major safety initiatives from 71.4% in Oct. 2004 to 98-100% in Feb. 2005
