Care Innovation and Transformation - Apply

AONE’s Center for Care Innovation and Transformation (CCIT) has released the application for the next two-year cohort of the Care Innovation and Transformation (CIT) initiative beginning in 2013 and continuing through 2014. The application process will be open until November 15, 2012. Both AONE members and non-members are encouraged to apply.  Hospital units invited to participate in the next CIT initiative cohort will be notified on November 20, 2012.

HOW TO APPLY
To apply for the 2013-2014 CIT initiative, please download the CIT application and the letter of commitment then submit both below, electronically. Applications and letters of commitment must be uploaded through the CCIT website by 5:00pm CST on November 15, 2012.
